WCHS Athletics Participation REQUIREMENTS

MCPS STUDENT - PARENT ATHLETIC PARTICIPATION INFORMATION & PERMISSION FORM explains the policies regarding participation in a sport, and permissions.

Health Inventory must be completed and signed by primary care physician PRIOR to participating in practice of any Churchill Sport. Physicals must be dated within 1 year

Brief Medical Information for the coach to carry at all games/competitions

Paid to MCPS prior to participation. Students who pay the fee are entitled to participate in one or more programs during the year.Online payment of student ECA fees for the 2010-2011 school year can be paid after July 15^th.

Forms are sent over the summer and can also be paid by check or money order to MCPS - ECA Unit, Division of Controller, 850 Hungerford Drive, Room # 161, Rockville, MD 20850. DO NOT SEND PAYMENT TO SCHOOL.
